Remote Diversion Specialist - NC Community Living (TCL)

Vaya Health is seeking a Diversion Specialist to implement the Diversion Referral and Screening (DRS) project and promote direct diversions for the Transitions to Community Living (TCL) for at-risk individuals. This mobile role involves fieldwork across NC, with documentation completed at home or an office. You will screen members, make referrals, provide education, and coordinate with hospitals, DSS, ACHs, and behavioral/physical health partners to ensure timely, person-centered care and #J-18808-Ljbffr Vaya Health
