The Licensed Clinical Social Worker (LCSW) plays a vital role in hospital settings, providing comprehensive mental health services primarily to adult clients. The LCSW works within a multidisciplinary team to deliver acute and in-patient care, ensuring effective treatment planning, case management, and coordination of care for a medium caseload of 15-30 clients. This position requires a strong commitment to clinical documentation and collaboration to support patient recovery and well-being.

13 Week contract with the option to extend

Hours: 08:00 - 16:30

 

Responsibilities

• Develop and implement individualized treatment plans
• Manage and coordinate care for in-patient and acute care clients
• Conduct thorough clinical assessments and documentation
• Collaborate regularly with multidisciplinary teams
• Provide case management services to adult clients
• Maintain accurate and timely client records
• Support care coordination within hospital settings

 

Preferred Qualifications

• 2+ years of experience in social work (Inpatient Case Management)
• Master's in Social Work (MSW)
• Licensed Clinical Social Worker (LCSW) or LSWAIC certification
• BLS certification
• Strong skills in clinical assessment and case management
• Excellent documentation abilities
• Effective interpersonal and problem-solving skills
• Epic Experience Required
