RN Care Navigator - Transitions of Care & Patient Support

Hunterdon HealthCare Partners seeks an RN Care Coordinator to join our outpatient team and coordinate care for high-risk patients to ensure safe transitions with an interdisciplinary group. The role requires RN licensure in New Jersey, BSN or equivalent, and 3+ years of acute care or home health experience.
