Employment Type: Permanent Hire

Compensation Range: $150K-$160K per Year

Location: Fort Sam Houston, TX

Work Schedule: Full-time, averaging 40 hours per week; 8–12-hour rotating shifts; outpatient coverage 0500–2200, seven days per week

Its pharmacy operations include high-volume ambulatory services requiring accurate dispensing, medication safety, clinical coordination, and consistent adherence to established pharmacy procedures.

Position Summary

The Staff Pharmacist – Outpatient provides pharmaceutical services within a high-volume ambulatory pharmacy environment. The pharmacist reviews and dispenses prescriptions, evaluates patient medication profiles, coordinates with prescribers, counsels patients, performs medication reconciliation, and supports safe and effective drug therapy.

The position requires flexibility to support rotating pharmacy operations based on staffing requirements. Pharmacists may rotate between ambulatory pharmacy or inpatient pharmacy sites as operational needs require.

Key Responsibilities

• Work collaboratively with multidisciplinary teams to optimize drug therapy.
• Receive and review prescriptions for the full range of pharmaceuticals, including controlled medications on the formulary.
• Determine medication type, strength, dosage, and quantity to be dispensed in accordance with accepted pharmacy practice and established standard operating procedures.
• Enter prescriptions into the outpatient pharmacy computer system and review patient profiles for allergies, drug-drug interactions, drug-food interactions, and duplicate orders.
• Coordinate with prescribers to clarify concerns involving medication dosing, interactions, or instructions.
• Obtain pharmaceuticals, prepare package labels, and visually inspect medications before dispensing.
• Verify the medication and printed container label against the original prescription for accuracy.
• Provide patients with instructions regarding the appropriate use of prescribed medications.
• Provide medication reconciliation for patients.
• Mentor pharmacy technicians and other pharmacy personnel working within the assigned area of responsibility.
• Respond to Code Blue events when designated as the Code Responder for an assigned shift.
• Rotate between ambulatory pharmacy or inpatient pharmacy sites when required to accommodate staffing needs.
• Maintain flexibility for operationally necessary schedule changes and assigned shifts.

Required Qualifications

• Bachelor's degree in pharmacy from a college or university accredited by the Accreditation Council for Pharmaceutical Education or applicable ACPE-accredited organization.
• Current, full, active, and unrestricted license to practice as a pharmacist.
• Minimum of one year of outpatient pharmacy experience within the previous two years post-graduation.
• Current Basic Life Support certification as required for the assignment.
• Ability to review prescriptions and patient profiles for dosing, allergies, interactions, duplicate therapy, and other medication-safety considerations.
• Ability to work onsite in Fort Sam Houston, Texas.
• Ability to work rotating shifts and support scheduling requirements based on operational need.

Preferred Qualifications

• Experience within a large hospital, federal healthcare, military healthcare, or similarly complex pharmacy environment.
• Experience dispensing and reviewing controlled substances.
• Experience with medication reconciliation and direct patient counseling.
• Experience collaborating with prescribers and multidisciplinary clinical teams.
• Experience mentoring pharmacy technicians or other pharmacy personnel.
