MPNN BH has over 60 distinct programs to aid and support all ages and many situations. Job Summary Under limited supervision, the employee is responsible for supervising staff and service activities for Assertive Community Treatment meeting the standards of fidelity. Routinely functions as a practicing clinician on the team. Evaluates, monitors and recommends procedures to ensure that the team operates efficiently and effectively using a recovery-based philosophy of care. Coordinates the development and completion of forms, policies and standard operating procedures as necessary. Supports the implementation and maintenance of electronic medical records and data collection. Directly supervises a team of interdisciplinary clinicians. Facilitates team meetings 4 to 5 days per week. Typical work hours are during the day with 24/7 on-call status via cell phone. This position may include some evening and weekend activities to meet program needs. Direct services, provided by the team, are predominantly home and community based. Serves as a liaison with organizations, services and stakeholders associated with ACT. Employee must exercise tact and courtesy in contact with staff, clients, families, public, and service providers. Critical component of job is to possess strong interpersonal skills. Reports to the Director of Community Based Services. Job Responsibilities • Monitor billing/revenue, individual staff productivity, and goals and outcomes for the ACT Program through the review and analysis of data. Utilizes the data to set future program standards of service in an effort to maintain program viability. • Provide clinical, quality management and administrative oversight for all ACT Program activities. Ensures appropriate services to individuals and compliance with documentation standards established by MPNN CSB and Medicaid (DMAS). Ensures licensing standards for the program are met. • Direct the day-to-day clinical operations of the ACT team including scheduling staff work hours to assure appropriate coverage for day, evening, weekend, and holiday shifts and on-call hours. • Utilize a stage-based treatment model that is non-confrontational, considers interactions of mental illness and substance abuse, and has individual-determined goals. Be comfortable with Harm Reduction approaches.
• Lead the daily organizational staff and treatment planning meetings; continuously evaluate the status of consumers and do appropriate planning and coordination of treatment activities to ensure immediate attention to their changing needs. • Direct and coordinate the individual's admission process and treatment, rehabilitation, and support services of the program in coordination with the psychiatric care provider. • Review referrals to ensure eligibility criteria are met. Schedule and coordinate the initial intake assessment; develop the initial treatment plan; assign the most appropriate staff to the individual treatment team (ITT); and provide clinical supervision of the development of the comprehensive treatment plan for each individual. • Direct and coordinate, for each individual, the comprehensive assessments of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g., self-care, living situation, nutrition, money management); and family structure and relationships. • Conducts recruitment activities to include screening applicants, conducting interviews, verifying credentials, hiring, work assignments, and orientation and performance supervision according to work rules, regulations, and policies; develop and implement staff orientation and training. • Supervise medical records management assuring maintenance of the electronic medical record in compliance with agency policies, Medicaid, and other third-party payment requirements;
• Train staff on medical record requirements; regularly review assessments, treatment plans, and progress notes written by the staff; and supervise individual staff for medical records mastery. • Carry out and document quality-assurance activities and reviews of use of program services. • Develop and maintain program policies and procedures and revise as necessary. • Comply with regulations for HIPAA and requirements related to mandated reporting of abuse and neglect. Ensures that staff are trained in all aspects of confidentially and human rights.
• Communicates program needs and related issues to the Director via supervision and other reporting mechanisms. • Acts as a liaison for the program with other agency departments and outside public and private agencies. • Conduct clinical assessments and provide direct clinical treatment, rehabilitation, and support services to individuals. • Perform on-call duty for crisis intervention and be available for staff supervision and consultation.
• Other Duties, as assigned Requirements Job Requirements Essential • Must pass all required Background Checks. • Must possess a valid Virginia Driver's License with reliable transportation and driving record acceptable to agency insurance carrier.
• Must obtain a TB test with satisfactory results within 30 days of employment, and yearly thereafter as required by licensing authority. Education and Experience • A Master's Degree in Nursing, Social Work, Psychiatric Rehabilitation, Psychology, Counseling or other Human Services and is a licensed mental health provider (LMHP)
• Requires three years' clinical experience working with populations with severe and persistent mental illness and/or co-occurring disorders and chronic disease management; progressive administrative experience to include staff supervision, training, and leadership.
• Strong computer skills are necessary.
• Supervisory and program management experience and licensure as a mental health professional is desirable. • Knowledgeable about the ACT Fidelity Model A $7,000 sign-on bonus will be paid in three(3) installments over a twenty-four-month(24) period.
