Overview The role involves serving as an Inpatient Social Worker within a multidisciplinary team, responsible for assessing, planning, advocating, and coordinating patient care from admission to discharge to ensure safe transitions and high-quality outcomes. Responsibilities Key duties include effective transition planning, oversight of length of stay, and maintaining patient safety and satisfaction through adherence to mission, values, and service standards. Qualifications Graduate of an accredited School of Nursing, with a minimum of a Bachelor’s Degree in Nursing. Current licensure as an RN in the state of practice is required, along with BLS (CPR) certification at hire or within 90 days. Experience in acute care hospital settings, with knowledge of Care Management or Utilization Management advantageous. Optional certification in Case Management is preferred.
