About the Position

Job Title: Licensed Clinical Social Worker Profession: Social Services Specialty: LCSW Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 12:00 PM to 8:30 PM Experience: Minimum 2 years of experience License: LCSW, LMFT, LPCC, or Licensed Psychologist Certifications: BLS, CPI Must-Have: Biopsychosocial assessment skills Description: Assess patients for the appropriate level of care. Provide support for individuals struggling with mental illnesses such as depression, psychosis, and substance use. Conduct biopsychosocial assessments and offer crisis intervention. Facilitate short-term counseling and link patients to community resources. Assist in planning for transitions of care for patients and their families. Provide psycho-education and lead support groups as needed. Consult on psychosocial aspects of care as part of an interdisciplinary team. Collaborate with multidisciplinary teams on discharge planning. Practice independently while providing patient support in adjusting to illness. Facilitate goals of care conversations with patients and families. Educate hospital staff and participate on relevant committees.
