Reports To: HIV Counseling & Testing Coordinator

Program: Outreach & Expansion

Salary Range: $45,000 - $55,000 annually

Position Overview

The Community Health Worker provides emotional, educational, and navigational support for clients facing barriers related to the social determinants of health. This role focuses on engaging, educating, and empowering community members to improve healthcare access, retention, and overall wellness.

Key Responsibilities

Outreach & Navigation

Conduct targeted community outreach to identify, engage, and re-integrate out-of-care individuals into health services.

Maintain ongoing communication with clients through check-ins, encouragement calls, and appointment reminders.

Accompany clients to medical, behavioral health, substance use treatment, and social service visits to provide on-site support.

Assist clients with appointment scheduling and coordinate transportation resources to ensure attendance.

Collaborate with healthcare providers to support Chronic Care Management plans.

Health Education & Care Coordination

Offer health literacy education, treatment adherence support, and ongoing emotional encouragement.

Assist clients in navigating documentation requirements for housing applications, social support programs, and health insurance.

Escort clients through initial medical or community service appointments to ensure clarity on care plans.

Track patient referrals and promote resource utilization across multidisciplinary care teams.

Represent program services to internal teams and external community partners.

Participate in required organizational meetings and ongoing professional development trainings.

Documentation & Compliance

Maintain accurate, timely, and compliant chart documentation following organizational standards.

Support data collection on client outcomes and participate in quality assurance chart reviews.

Use health information systems to track productivity and drive quality improvement.

Safety & Operations

Follow CDC-aligned standard precautions.

Maintain active working knowledge of safety procedures, including the Emergency Code System and Continuity of Operations Plan.

Adapt flexibly to shifting organizational needs and perform additional project duties as assigned.

Qualifications & Requirements

Education: High school diploma or equivalent required; Associate or Bachelor's degree preferred.

Experience: Minimum of 1 year of experience in community outreach and working with at-risk populations.

Language Skills: Bilingual proficiency in English/Spanish or English/Haitian Creole preferred.

Technical Skills: Computer literacy, including proficiency in Microsoft Office (Word, Excel) and health information systems.

Core Competencies: Strong interpersonal, organizational, and problem-solving skills with a proven ability to engage diverse, multicultural populations.

Work Logistics: Self-motivated, detail-oriented, and flexible to travel locally and work adaptable hours as needed.

Physical Demands: Standard office and field environments. Ability to operate office equipment, perform light lifting/bending, and communicate clearly.
