Our platform automates charting and compliance so you can focus on high-quality senior care.We're seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador.Routine & urgent assessments; chronic disease managementPost-discharge follow-ups, med reconciliation, labs/diagnosticsCoordinate with facility staff, specialists, families/POAsSame-day documentation; CMS/regulatory compliancePart-time position with a structured weekday schedule. You'll spend 3 days per week delivering care, with some days on-site across Stockton, Modesto, and Amador (strategically clustered routes designed to minimize travel and maximize time with patients) and other days via telehealth.
