Registered Nurse - Home Health

Location Caledonia County, Vermont

Assignment Duration This is a 13-week contract position.

Schedule Days, Monday to Friday, with possible weekend shifts.

Job Description As a Registered Nurse providing care through home health services, you will deliver a full range of professional nursing services to clients of all ages. Your role will encompass admission processes, evaluations, and the development of tailored care plans. Working collaboratively with a dedicated team of homecare professionals, you will ensure the highest level of coordinated care for clients throughout Caledonia and southern Essex counties.

Responsibilities Conduct patient assessments and admissions.

Develop and implement individualized care plans.

Administer medications and treatments as prescribed.

Collaborate with paraprofessionals and other healthcare team members.

Monitor patient progress and adjust care plans as necessary.

Educate patients and families on health conditions and care plans.

Required Experience / Certifications / Licensure Current Vermont Registered Nurse (RN) license.

Minimum of one year of prior clinical experience required.
