- Conduct evaluations for patients in both inpatient and outpatient settings and create personalized treatment plans
- Implement physical therapy interventions focusing on mobility, strength, balance, and overall function
- Collaborate with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
- Educate patients and their families regarding therapeutic objectives, discharge guidelines, and home exercise plans
- Document evaluations, treatments, and patient progress in line with facility standards and regulatory requirements
- Engage in team rounds and interdisciplinary meetings to uphold coordinated care
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
- State licensure as a Physical Therapist
- CPR certification is preferred
- Specialized certifications (e.g., neurological, geriatric) are advantageous but not necessary
- Previous clinical experience is highly valued
- Ability to treat patients with intricate medical and mobility challenges
- Excellent communication and teamwork capabilities
- Comfortable navigating digital charting and clinical documentation systems
