Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum of 2 years of acute care experience required. License: Compact license required. Certifications: Current certifications relevant to case management. Must-Have: - Acute care experience - Case management experience - Experience with HMOs, IPAs, and advance directives - Ability to conduct needs assessments and discharge planning Description: The position requires a registered nurse specializing in case management. Candidates must have current acute care experience within the last year. Weekends are required, with scheduling every other weekend. There will be no call requirements. Travelers may be expected to float between similar units based on department needs. Local candidates are accepted for this position. All requests for time off must be submitted upfront during the application process. The role involves significant patient coordination and support for various patient age groups. Candidates must be knowledgeable of HIPAA guidelines and CMS regulations. The facility provides resources to assist with housing for travelers. No contact should be made with facility representatives unless it has been pre-arranged. References from previous employers are required during the compliance process. The candidate should be flexible and committed to the assigned schedule. Travelers may float between nearby facilities if required.
