Program Supervisor II

As the Program Supervisor II for the Blackburn Recovery Program (BRP), you will play a pivotal leadership role in an innovative, mission-driven program dedicated to helping individuals achieve lasting recovery and housing stability. You will oversee daily operations, mentor and support a multidisciplinary team, and help drive services that integrate housing, healthcare, recovery support, and employment resources under one roof. Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty and addictions. The Blackburn Center combines supportive housing with an on-site health care clinic, pharmacy and employment services. Blackburn Center Apartments is a six-story apartment building located in the Hazelwood neighborhood of SE Portland, consisting of 90 single-room occupancy units and 34 studio units. Schedule: Monday - Friday, 8:00am - 5:00 PM Location: Blackburn Clinic, 12121 E. Burnside St. Portland Compensation: $57,000 - $80,000 Final comp will be determined based on relevant experience, education, credentials/licensure, and internal equity. MINIMUM QUALIFICATIONS: High School diploma or GED required. Bachelor’s degree in a relevant field preferred. Two years of experience working in supportive housing, behavioral health, social services, or related environment with one year of supervisory or team lead experience required. Must be able to demonstrated excellent computer skills. Must be able to work in all CCC buildings. Some CCC buildings have restrictions due to criminal background and/or active probation and parole. Ability to adhere to Central City Concern’s drug-free workplace which encourages a safe, healthy, and productive work environment and complies with the Drug-Free Workplace Act of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol. Must pass a pre-employment drug screenand background check. May be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern. Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects, and climb stairs several times a day, with reasonable or no accommodations. Must adhere to agency's non-discrimination policies. Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, cultural backgrounds, lifestyles, and sexual orientations, and treat everyone with respect and dignity. Ideally has a CADC I, CRM, PWS, or QMHA Valid driver’s license preferred. If required to drive, must pass a DMV background check, and be designated an “acceptable” driver as outlined in Central City Concern’s Fleet Safety Policy. If required to drive, must pass an initial driver’s training within 60 days of being an approved driver and continued recertification training. ESSENTIAL DUTIES AND RESPONSIBILITIES: Work as a member of the Blackburn Center multi-disciplinary team (housing, maintenance, clinical) to deliver comprehensive, client centered, and trauma informed integrated services to the Blackburn Early Recovery and Blackburn Stabilization clients. Actively participate in the integration of services at the Blackburn Center and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines. Proactively identify opportunities for improvement within Blackburn Center workflows and logistics. Participate in review of Blackburn Key Performance Indicators; review and contribute to continuous improvement opportunities and plans. Build understanding of and appreciation for diversity among Blackburn Center clients and support their connection to culturally responsive and culturally specific services as requested. Responsible for the supervision of the Blackburn Recovery Program staff. Supervise Case Management, Peer Support Specialist, and Housing Specialist Staff. Communicate with internal and external service and referral partners, treatment providers, and Portfolio Manager. Ensure compliance with the program requirements, including ensuring proper record keeping as per donor and internal best practices. Ensure BRP team programmatic best-practices, including conducting of intake interviews, following Peer Support and Case Management best practices, and explanation of BRP programmatic expectations to clients. Ensure BRP colleagues are supporting clients, in collaboration with Community Health Workers, Health and Assessment Team, to transition clients to a more appropriate housing or treatment situation when appropriate. Ensure weekly goals and activities for each client are established and updated by the BRP case managers and peer support specialists. Ensure program forms reflecting the current needs of the program are in place and audited regularly. Ensure staff are reflecting work with BRP clients with accurate and timely updates to service notes, case plans, and other key case management documents identified by Central City Concern Attend clinical staff meetings and case conferences to ensure alignment and effective communication between partners working with BRP clients Supervise and monitor all administrative responsibilities of staff in the BR program. Manage program referrals and outreach to referring partners to fill units in manner aligned with program contract Monitor client utilization of assigned unit and overall occupancy of the program units. Work with direct supervisor to monitor program budgets, and spend discretionary funds for program, staff, and client care . Provide reports and other paperwork as required. Participate in compliance, quality assurance and quality improvement activities as directed. Be available by cell phone 24/7 for all emergencies. Gather, edit, and refine program policies and procedures and impact data. Help to keep files organized and updated. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee. Perform related duties as assigned, within the scope of the role. SKILLS AND ABILITIES: Demonstrated basic knowledge and skills in chemical dependency, mental health, and issues of homelessness. Ability to work with ex-offenders in groups and as individuals. Ability to work as a team member and to establish priorities. Ability to display a non-judgmental attitude, and a willingness to learn new skills. Ability to work with clients having severe mental health issues, life threatening physical health issues, MR/DD issues and dangerous psychopathy. Demonstrated ability to work without supervision. Demonstrated knowledge of community and social agencies, and self-help groups. Ability to supervise all support staff members. Sufficient manual dexterity and physical ability to perform assigned tasks. Ability to communicate clearly and concisely both verbally and in writing. Ability to manage time and meet deadlines. Ability to understand and follow verbal and written instructions. Ability to work courteously with the public, tenants, and co-workers. Ability to maintain accurate records and necessary paperwork. Ability to learn and apply training instruction. Knowledge of de-escalation methods or ability to be trained in de-escalation methods. Ability to effectively communicate with clients, co-workers, corrections personnel, police, courts merchants, the public at large and supervisor. Ability to provide leadership. Ability to provide advocacy, as appropriate. Professional maintenance of appropriate boundaries. Ability to establish and maintain cooperative working relationships with those contacted during work.
