Job Description:

This RN position is within the Inpatient Palliative Care unit, responsible for managing consults and providing palliative care services. The role involves working closely with patients and families, conducting assessments, and supporting end-of-life care discussions.

Responsibilities:

• Develop daily patient lists and review patient charts.
• Assess patient needs and conduct Goals of Care discussions.
• Educate patients and families on disease processes, Advance Directives, and POLSTs.
• Facilitate family meetings and provide end-of-life support.
• Participate in daily rounds and coordinate with physicians and healthcare teams.

Requirements:

• 2-5 years of recent experience in palliative or hospice settings; ICU or oncology experience preferred.
• Strong acute care, clinical thinking, and communication skills.
• Flexibility, organizational skills, and a desire to serve patients and families.
• BLS certification required; ELNEC preferred.
