Job Description\n Care Career is seeking a travel nurse RN Home Health for a travel nursing job in Green Bay, Wisconsin.Job Description & Requirements\n \n Specialty: Home Health\n Discipline: RN\n Start Date: 10/05/2026\n Duration: 13 weeks\n 36 hours per week\n Shift: 12 hours, days\n Employment Type: Travel\n \n Hospice RN – Home Health | Weekend Program\n Location: Green Bay & Oshkosh, WI markets\n Shift: 8:00 AM–8:00 PM, Friday–Sunday only\n Contract: 13 weeks\n Weekend: Every weekend – Fri/Sat/Sun schedule\n Setting: Home Hospice – visits patients in their homes\n EMR: Epic (Rover) / Remote Client\n Scrubs: Navy Blue\n Service Area\n \n Green Bay, Oconto, Abrams, Suamico, Howard, Pulaski, Bonduel, Nichols, Black Creek, Seymour, Hobart, De Pere, Bellevue, Luxemburg, Casco, Algoma, Kewaunee, Denmark\n Kaukauna, Oshkosh, Hortonville, Appleton, Neenah, Winchester, Winneconne, Omro, Ripon, Rosendale, Brandon, Markesan, Oakfield, St. Peter, Mount Calvary, Lomira\n \n Job Details\n \n Home Hospice RN providing care in patients' homes.\n Patient population includes failure to thrive, advanced cancers, end-stage COPD, terminal illness, and end-stage disease processes.\n Perform focused assessments, symptom management, wound care, Foley exchanges, feeding tube troubleshooting, post-fall assessments, and death visits.\n May perform PRN after-hours visits for symptoms, falls, death visits, etc.\n RN caseload: 0–6 depending on call-ins.\n May occasionally assist slightly outside assigned territory if closer to a patient; generally no more than 1 hour from home territory.\n Epic Rover/Remote Client\n Orientation: 3 shifts (1 week) with an SCT nurse\n Lead RN available 24/7 for support\n Free parking\n \n Requirements\n \n 2+ years Home Health or Home Hospice experience\n First-time travelers accepted if they have home-setting case management experience\n BLS\n Active Wisconsin RN license\n Active Driver’s License\n Own vehicle + auto insurance\n \n About Care Career\n \n Care Career brings together a portfolio of leading healthcare staffing organizations, each delivering specialized talent solutions across the industry.
