Key Responsibilities Assess residents and monitor changes in condition, symptoms, and vital signs. Administer medications and treatments according to physician orders and established standards. Develop, implement, and update individualized resident care plans in collaboration with the interdisciplinary team. Communicate changes in resident condition to physicians, leadership, and other appropriate team members. Educate residents and families regarding care, treatments, and health management. Maintain accurate, timely, and compliant clinical documentation. Coordinate resident care with nursing assistants, physicians, therapy, and other departments. Follow infection prevention, safety, regulatory, and facility standards. Provide compassionate care while maintaining resident dignity, privacy, and individual preferences. Demonstrate sound clinical judgment and respond appropriately to resident needs and changes in condition.
