Community Health Navigator - Lorain County
Cleveland Clinic Lerner Research Institute
Join the Cleveland Clinic Health System as a Community Health Worker in Lorain County. You will connect diverse, underserved populations to health and social services, working closely with medical providers to improve patient care and outcomes. You will visit patients in the community and their homes, build trusting relationships, educate on health resources, and support navigation of care plans and programs. This role runs Monday–Friday 8:30 a.m. – 5:00 p.m., with occasional events. #J-18808-Ljbffr Cleveland Clinic Lerner Research Institute
