Hospice Intake RN: Compassionate Care Advocate

The Community Hospice is seeking a Per Diem Hospice Registered Nurse (RN) for Intake/Admissions in Albany. You will join the Central Intake & Admissions Team and support patients and families during the hospice transition, with day shifts and part-time hours. Responsibilities include serving as the liaison among stakeholders, coordinating referrals, and initiating care plans. Eligible candidates hold an NY RN license, ASN, and preferably a BSN, with home care and hospice experience a plus. #J-18808-Ljbffr IHA
