RN Hospice Case Manager

We are seeking an experienced RN Hospice Case Manager for a contract assignment in Fort Worth, TX. The ideal candidate will have prior hospice experience and proficiency with Homecare Homebase (HCHB).

Schedule: Monday–Friday, 8:00 AM – 5:00 PM

Pay: $45-50/hr

Contract Duration: 13 Weeks

Guaranteed Hours: 40 Hours/Week

Role Overview

We are seeking an experienced RN Hospice Case Manager for a contract assignment in Fort Worth, TX. The ideal candidate will have prior hospice experience and proficiency with Homecare Homebase (HCHB). This role focuses on delivering compassionate, patient-centered hospice care, coordinating services across the interdisciplinary team, and supporting patients and families throughout the end-of-life journey.

Key Responsibilities

• Conduct comprehensive patient assessments to identify physical, psychosocial, emotional, spiritual, environmental, and safety needs.
• Develop, implement, and update individualized care plans based on patient and family needs.
• Coordinate care with physicians, social workers, chaplains, aides, and other interdisciplinary team members.
• Participate in interdisciplinary team (IDT) meetings to review and revise patient care plans.
• Assess and manage pain and symptoms while developing individualized comfort care strategies.
• Provide education to patients, families, and caregivers regarding disease processes, medications, treatments, and end-of-life care.
• Support patients and families through the hospice journey while maintaining dignity, comfort, and quality of life.
• Document all assessments, interventions, care plans, and outcomes accurately within the electronic medical record.
• Ensure compliance with hospice regulations, accreditation standards, and organizational policies.
• Advocate for patients and families while supporting cultural, spiritual, and personal preferences.
• Coordinate community resources and support services as needed.
• Promote effective grieving, self-determined life closure, and family-centered care.

Required Qualifications

• Active RN License (Texas or compact state license).
• Minimum 1 year of recent Hospice Nursing experience.
• Previous Hospice Case Management experience required.
• Experience with Homecare Homebase (HCHB) required.
• Current BLS Certification.
• Strong assessment, care coordination, and patient education skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage a caseload effectively.
