Job Details

• Weekly Estimated Pay: $2,500 - $2,630
• Location: Newton, MA (02459) – North Region Scheduling; Boston-area travel (Back Bay, Beacon Hill, Cambridge, Charlestown, Fenway, South Boston, etc.)
• Assignment Duration: 13 weeks (May 31, 2026 – August 29, 2026)
• Hours: 40 hours per week
• Shift: 8 Hour Days (08:00 AM – 04:30 PM)
• Weekend Rotation: Once every 6 weeks
• Employment Type: Travel / Travel RN – Home Health

Job Requirements (Home Health RN / Case Management)

• Active RN license in Massachusetts (MA)
• Minimum 1 year of certified Home Care / home health nursing experience
• Minimum 1 year of Case Management experience
• Ability to travel to patient cases as assigned throughout the North Region and greater Boston area (including Back Bay, Beacon Hill, Boston, Cambridge, Charlestown, Chinatown, Fenway, Financial District, Kenmore, North End, Prudential, South Boston, South End, West End)
• Strong electronic documentation skills and experience with EHRs for timely, accurate charting
• Excellent interdisciplinary communication and patient education skills
• Required onboarding credentials (background check, health screenings, skills checklist, references, resume, etc.)

Homecare RN Responsibilities (Home Health / Clinical Case Management)

• Assess patient conditions in the home setting and develop individualized, patient-centered care plans
• Provide ongoing education, support, and care coordination for patients and their families
• Deliver home health nursing care in accordance with organizational policy and community health standards
• Perform admissions, care planning, and case management duties to support continuity of care
• Maintain thorough, timely electronic documentation in the EHR to support quality and compliance
• Collaborate with interdisciplinary teams (physicians, therapists, social workers) to ensure comprehensive patient care
• Participate in weekend rotations and flexible scheduling as assigned
