Licensed Practical Nurse - Operating Room

We are seeking a Nurse Practitioner for an in‑person, full‑time role in San Jose, CA. Key Duties Include Assessing, diagnosing, and treating patients across all ages. Prescribing and managing medications within scope, including dispensing if applicable. Creating personalized treatment plans in collaboration with patients and care teams. Performing physical exams, reviewing histories, and evaluating clinical presentations. Providing preventive care, chronic disease management, and behavioral health support. Educating patients on disease processes, medications, and prevention strategies. Coordinating referrals, collaborating with multidisciplinary teams, and supporting integrated care. Participating in on‑call rotations, maintaining accurate medical records, and engaging in quality improvement initiatives. Supporting care coordination, patient follow‑ups, and community health activities. Qualifications Include Master's degree or higher in Nursing. Active California Nurse Practitioner license or eligibility. Family Nurse Practitioner (FNP) preferred. Current DEA registration. At least 2 years of clinical experience in family medicine, internal medicine, or pediatrics. Strong knowledge of primary care, preventive medicine, and community health principles. Excellent communication, critical thinking, and patient education skills.
