Social Worker/Care Companion - Framingham, MA Job Summary Carease Health - MA is seeking a dedicated and compassionate Social Worker/Care Companion to join our team! As a Social Worker/Care Companion, you will work closely with our experienced clinical team to provide comprehensive preventative care to nursing home patients. This is a high-volume setting, requiring strong organizational skills and the ability to work efficiently in a fast-paced environment. Our ideal candidate will have at least 1 year of experience as a Medical Assistant or Certified Nursing Assistant (CNA) and be skilled in taking vital signs. Responsibilities Conduct interviews and assessments with patients to identify social, emotional, and psychological needs Develop and implement individualized care plans to promote patient well-being and independence Provide emotional support and counseling to patients and their families Collaborate with healthcare providers to ensure coordinated care and address patient needs Monitor patient progress and adjust care plans as needed Participate in interdisciplinary team meetings to share information and develop strategies for patient care Keep accurate and detailed records of patient interactions and care plans Requirements Associate's degree in Social Work or related field At least 1 year of experience as a Medical Assistant or Certified Nursing Assistant (CNA) Skilled in taking vital signs Strong communication and interpersonal skills Ability to work efficiently in a fast-paced environment Reliable and able to maintain a stable schedule, Monday through Friday, with no weekend work Driver's License (Required) Certified Medical Assistant (Required) Benefits Competitive salary range: $57,000 - $59,000 ANNUALLY Monday to Friday schedule with no weekend work Opportunities for growth and professional development #J-18808-Ljbffr Careasehealth
