Therapy Coordinator
Job Summary:
Orion Home Health is seeking a dedicated and compassionate Therapy Coordinator to join our team in Morton Grove, IL. Key Responsibilities:
Manage Therapy Schedules: Coordinate patient therapy schedules, ensuring efficient and effective use of time
Communicate with Care Teams: Collaborate with interdisciplinary teams, including healthcare providers, therapists, and other support staff to deliver high-quality patient care
Scheduling and Coordination: Schedule therapy sessions, appointments, and evaluations; coordinate transportation, equipment, and other necessary resources
Patient Assessment and Care Planning: Assess patient needs and develop care plans in collaboration with healthcare providers
Ensure Compliance: Maintain accurate and up-to-date patient records, adhering to regulatory and organizational standards
Requirements:
Experience: 1-2 years of experience in a healthcare setting as a coordinator
