The elements are: Integrity Personability Transparency Summary Vighter is recruiting Registered Pharmacists for positions at Luke Air Force Base (LAFB), AZ. Registered Pharmacists dispense prescription medications and provide information to patients about the drugs and their use. They also advise physicians and other healthcare workers on the selection, dosage, interactions, and side effects of medications to treat health problems. Key Responsibilities Assess patient’s response to drug therapy and plan drug therapy based on physician established diagnoses. Order and evaluate laboratory tests necessary to evaluate drug therapy effects and outcomes. Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states (e.g., hypertension, hyperlipidemia, anticoagulant, diabetes, asthma, refill clinics, etc.), in cooperation with the medical staff. Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics (e.g., aminoglycosides, phenytoin, antithrombotic). Administer prescription or non-prescription drugs according to established agreements or MTF protocols. Assess metabolic needs and order therapeutic enteral or parenteral nutrition products in the inpatient setting. Evaluate medical and medication histories for drug-related problems and adjust drug therapy accordingly. Consult with other healthcare providers (e.g., physicians, dietitians, nurses, physical therapists, etc.) about patient treatment needs or options. Perform a full range of pharmacist procedures. Review patient profiles at the time of dispensing in order to closely monitor medication therapy. Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients. Provide medication and healthcare related in service training to clinical staff on new medications on the market; appropriate use of medications; comparison of current medication therapies; and other topics as deemed appropriate by the department head. Consult with other specialty practitioners to provide optimal patient pharmaceutical care. Experience & Education Doctorate, Pharm.D. is required. Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE). Internship/Residency: Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy. Must meet clinical competency requirements specified in the Service‑specific credentialing instructions. Qualifications Current, full active, and unrestricted licenses to practice as a pharmacist. Knowledge, Skills and Abilities Communication and Consultation; Global and Cultural Awareness; Ethical Practices. Moderate proficiency in common Microsoft Office Suite programs, including Apple (iPhones/iPads), iOS operating system, Pages and Numbers applications (iOS), Microsoft Word, Excel, Outlook, and SharePoint. Functional proficiency and ability to document patient‑centered healthcare in the required format. Knowledge of regulations (HIPAA/Privacy Act) regarding the confidentiality of patient medical records and information as well as Personally Identifiable Information (PII). Duty Hours / On‑call Requirements Generally speaking, this is a day shift position. The work schedule for each labor category is as follows: 0700-1730 Monday thru Friday. NTE 8 hours per day and 40 hours per week. With exception, when directed by the Government, the Pharmacy staff shall remain on duty to complete patient treatment where lack of continuity of care would jeopardize patient health. This provision is not intended to apply to the time required to complete routine tasks (e.g., completion of paperwork or routine administrative tasks at the end of a shift) which are to be completed as part of the normal work schedule. Physical Requirements Must be able to walk, stand, bend, and/or sit for more than 8 hours per day. Must be able to use an elevator or walk up and down stairs. Must be able to lift up to 50 pounds and carry up to 10 feet. Must be able to routinely grasp or handle objects, use finger dexterity, bend elbows/knees and reach above and below shoulders. Must be able to read and interpret handwritten and typewritten print. Must be able to communicate by voice and detect sound by ear. Must be willing to work day, night, weekdays, weekends, holidays, and overtime if necessary. Must be able to perform duties in an indoor and/or outdoor environment. Must be able to pass a criminal background investigation. Candidate must not have been convicted of, pled guilty to, pled nolo contender to, or received probation prior to judgement for any crime other than a minor traffic violation, the record of which has not been expunged. Must undergo Urine Drug Screening (UDS). Must be a Citizen of the United States or legally authorized to work in the United States of America.
