Hospice RN Case Manager — Flexible Schedule, In-Home Care

St. Mary's Home Care and Hospice in Bucks County, PA is seeking a full-time Hospice RN Case Manager to provide professional nursing care to hospice clients as prescribed by the physician and in line with our policies. The RN will conduct comprehensive assessments, develop and implement care plans, coordinate with an interdisciplinary team, and oversee patient outcomes. Collaboration with physicians and Family/Caregivers is essential to deliver compassionate, home-based care. #J-18808-Ljbffr IHA
