Together, we're building a stronger, more connected healthcare system.Our Nurse Practitioner will perform comprehensive health assessments which include but not limited to the following:Obtain history and perform physical examinationComplete review of medicines and related instructionsReview home safety and psychosocial needsFulfill documentation needs for STAR and HCC measuresObtain and document all pertinent information in patient's charts and case management notes on same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissionsInstruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergenciesMaintain patient safety and confidentialityUniversal precautions utilized in all patient carePrescribe/order procedures, treatments and medications within scope of practiceDevelop high level of trust and intimacy with patients and familiesThe Skills, Experience & Education You Bring:CA Licensed Nurse Practitioner1+ Years of Experience with Primary CareThe Base Salary for this position is $135,000 - $155,000/year. Final determination of a successful candidate's starting pay will vary based on several factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements.
