The Texas Behavioral Health Center is anewly completed state-funded psychiatric hospital opening in Dallas’ Southwestern Medical District that is owned by the Texas Health and Human Services Commission (HHSC) and operated by UT Southwestern Medical Center. When fully operational, the 505,000-square-foot facility will house 292 beds – 200 for adult patients and 92 for children and adolescents. As DFW’s first state‑funded behavioral health hospital, it is designed to deliver state‑of‑the‑art, patient‑centered care in a modern and healing environment. The center provides comprehensive behavioral health treatment and features private patient rooms, abundant access to natural light and nature, and thoughtfully designed spaces that promote recovery. The hospital will also provide training to all behavioral health disciplines. JOB DUTIES

Participates in multidisciplinary rounds and proactively determines patient needs for discharge transitions including transportation, DME, LTAC, SNF, home health agencies and hospice. Utilizes crisis intervention and problem-solving skills to facilitate and enhance patient and family's ability to meet acute and ongoing medical and psychosocial needs. Demonstrates ongoing familiarity with community resources to facilitate positive patient and family outcomes agencies to assure appropriate client access. Maintains effective documentation including tracking and referral information; consistently meets department and UTSW documentation standards. Identifies and reports situations or cases to appropriate governmental agencies as legally required and in compliance with organizational policies and procedures. Serves as an active participant of the health care team, communicating effectively with other health care providers, patients, families, other hospital personnel, and visitors. Identifies the need for professional growth and seeks appropriate development opportunities. Provides education to staff, patients and community resource affiliates. Maintains system for tracking placement of patients after discharge on monthly basis. Has knowledge of Department and Unit Performance Improvement program and is aware of expected participation. Identifies need for professional growth and seeks development opportunities. Provides education to staff, patients, and community resource affiliates. Performs other duties as assigned.
