Work Environment

This position is full-time and focused on onsite home care, where you will be deployed to patients' residences across Yonkers, New York. Here, you will engage with patients and their caregivers in a familiar environment, facilitating comfort and maximizing rehabilitation outcomes. Working standard shifts from Monday to Friday, you will enjoy a stable work-life balance while contributing directly to patient care.

Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians to develop individualized treatment plans that align with their specific rehabilitation goals.
• Administer a variety of therapies and interventions, adapting approaches for orthopedic, pediatric, neurologic, and sports medicine cases, ensuring comprehensive care tailored to each patient's needs.
• Collaborate effectively with a multidisciplinary team, using results-driven methods and evidence-based practices to enhance patient outcomes.
• Provide high-quality therapeutic care directly in patients' homes, applying various techniques and procedures in accordance with physician directives to improve function and condition.
• Educate patients and their families about therapeutic techniques and home exercise programs, empowering them with the knowledge to continue their rehabilitation beyond therapy sessions.
• Meet productivity and utilization goals while maintaining high standards of patient care and satisfaction.

Patient Population / Client Focus

Our Physical Therapists work with a diverse population of clients, including those recovering from surgery, managing chronic conditions, and seeking to improve functional abilities post-injury. You will foster meaningful connections with each patient, understanding their unique challenges and providing solutions to help them achieve their personal rehabilitation goals.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited APTA or CAPTE-approved program.
• Valid State License in Physical Therapy to practice in New York.
• Current Basic Life Support (BLS) certification from the American Heart Association.
• Strong verbal and written communication skills; ability to convey information effectively to patients and coworkers.
• Proficiency with word processing and computer applications to maintain patient records and reports.
• Demonstrated ability to cultivate strong, trusting relationships with patients and their families.
• A genuine desire to connect positively with individuals and contribute to community well-being.

Preferred Experience

• Prior experience in home health settings or direct patient care roles will be considered an advantage.
• Hands-on experience with diverse patient populations, including specific exposure to pediatric or neurological therapies.
• Proven capability in using evidence-based methodologies for patient outcomes improvement.

Compensation and Benefits

We offer a competitive salary that ranges from USD 100,000 to 120,000 annually, reflecting your experience and expertise. In addition to a strong base salary, our benefits package includes:
