Job Description

The Outreach and Enrollment (OE) Program Coordinator directs and manages NATIVE HEALTH’s Connecting Kids to Coverage (CKC) Outreach and Enrollment program and the SNAP CMS program. The position oversees a team of Outreach & Enrollment Specialists (OES) across all NATIVE HEALTH locations; ensures program, grant, budget, and reporting requirements are met; and serves as the primary liaison to the funder, community partners, and stakeholders. The coordinator is also a visible community advocate for access to enrollment assistance and education. The Outreach and Enrollment Program Coordinator reports to the Health Services Director.

Evaluation: Performance of this job will be evaluated in accordance with provisions of the NATIVE HEALTH Employee Handbook on evaluations.

KEY ACCOUNTABILITIES:

1. Program Leadership and Operation: Provides direction and oversight of the planning and operation of the Connecting Kids to Coverage (CKC) Outreach and Enrollment in alignment with NATIVE HEALTH’s mission, values and strategic plan.

Measure of Success:

  1. Ensures cooperative agreement goals, objectives, activities, and evaluation requirements are met.
  2. Coordinates outreach and enrollment services with other NATIVE HEALTH programs and external partners, including schools, school districts, and public and private organizations
  3. Ensures clients and patients have optimal access to enrollment assistance and education.
  4. Coordinates a community advisory board that represents the populations served by the funded program and provides ongoing guidance.
  5. Maintains collaborative relationships with councils and boards, including the National Council of Urban Indian Health (NCUIH), National Indian Health Board (NIHB), and Arizona Alliance for Community Health Centers (AACHC).

2. Staff Leadership and Development: Leads, coaches, mentors, and supervises staff to support engagement and achievement of organizational and cooperative agreement goals.

Measures of Success:

  1. Ensure staff understand and demonstrate NATIVE HEALTH’s mission and values.
  2. Provides appropriate orientation, training, and ongoing development.
  3. Ensures clients and patients have optimal access to enrollment assistance and education.
  4. Ensures staff maintain current knowledge of Medicaid (Arizona AHCCCS), the Children’s Health Insurance Program (CHIP), and the Health Insurance Marketplace through required training, seminars, and workshops.
  5. Monitors productivity benchmarks, program deliverables, and core competencies.
  6. Provides timely, constructive performance feedback.
  7. Ensure staff attain and maintain required Certified Application Counselor and Community Health Worker (CHW) certifications.
  8. Completes all performance evaluations for all direct reports.
  9. Engage staff in delivering a patient experience that surpasses expectations.

3. Program Compliance, Budget, and Performance: Manages program compliance, budget performance reporting, and outcome evaluation.

Measures of Success:

  1. Monitors program spending and make programmatic adjustments to ensure budget conformity.
  2. Uses, data, reports, and trends to evaluate, manage, and improve services.
  3. Completes and submits all required quarterly, annual, financial, and evaluation reports on time.
  4. Monitors goals and objectives and implements and evaluates strategies to increase program activity and effectiveness.

4. Outreach, Enrollment, and Community Partnerships: Ensures accountable delivery of outreach enrollment assistance, and education to clients and communities.

Measures of Success:

  1. Identifies community events to provide community outreach in support of program goals and objectives.
  2. Oversees coordination of event logistics including staff assignment, event set up and tear down, providing outreach and marketing materials and all necessary elements for a successful event.
  3. Serves as the primary liaison to existing and prospective partner agencies and coordinates outreach and enrollment activities.
  4. Coordinates and collaborates with Outreach and Enrollment team members, patients, and families to meet program requirements.
  5. Implement national enrollment initiatives.
  6. Produces multimedia marketing and educational materials for clients.

5. Supports the organization as needed .

KNOWLEGE, SKILLS, AND ABILITIES:

  1. Ability to supervise, mentor and coach personnel and provide a positive team-oriented environment.
  2. Expert knowledge of eligibility requirements for Arizona Medicaid, Children’s Health Insurance Program (CHIP), and Health Insurance Marketplace.
  3. Knowledge of the cultural, social, and economic background of the American Indian/Alaska Native (AI/AN) and minority populations.
  4. Experience in grants and project management.
  5. Ability to analyze and evaluate data and reports.
  6. Strong customer service skills and the ability to handle difficult situations.
  7. Flexibility to work late hours or weekends when necessary.
  8. Ability to work effectively with government departments, community organizations, agencies, and the general community.
  9. Proficiency with computer-based applications such as MS 365, Adobe, and the health-e-Arizona Plus system.
  10. Effective oral and written communication skills.
  11. Cultural competence and the ability to work with a diverse workforce and client population.
  12. Openness to change and willingness to lead change within the organization.
  13. Professional integrity and an excellent work ethic.
  14. Ability to organize, plan, work independently, take initiative, manage competing priorities, and address concerns and issues effectively.
  15. Ability to network, develop meaningful collaborations, and coordinate activities across multiple programs and organizations.
  16. Knowledge of community resources.
  17. Ability to protect confidential information and maintain the trust of patients, families, and the agency.

Qualifications

  1. A bachelor’s degree in business, health administration, public health, or a related field is required. A master’s degree is preferred.
  2. Minimum of two years of experience relative to Medicaid and CHIP, outreach, education and enrollment services.
  3. Minimum of two years of supervisory and mentoring staff experience is preferred.
  4. Two years of experience with fiscal oversight of government programs, contracts and/or grant preferred.
  5. Must maintain Certified Application Counselor with CMS and remain current on Medicaid, CHIP, and marketplace training.
  6. Must obtain a Community Health Worker (CHW) certification.
  7. Must maintain active enrollment as a Health-e-Arizona Plus Assistor.
  8. Must have a broad understanding of Medicaid, CHIP, and outreach and enrollment best practices, strategies, and community resources.

Standard Employment Requirements:

  1. Must possess a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona driver’s license during employment. Must be insurable under NATIVE HEALTH’s liability auto policy.
  2. Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
  3. Must pass substance abuse testing upon employment and submit to random testing during employment.
  4. Must provide an updated immunization (IZ) record at the time of employment.
  5. Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
  6. Must obtain Basic Life Support (BLS) certification within (90) calendar days of employment and maintain certification during employment.
