We are seeking a compassionate advocate to join our team as the Case Manager Senior - RLRA Housing Retention . In this role, you will apply strengths-based practice to empower individuals as they transition to independent community living, ensuring that stable housing remains the foundation for their continued wellness and recovery.
This highly independent Case Manager Senior - RLRA Housing Retention role serves as the primary anchor for housing stability, specifically supporting Multnomah County Regional Long-Term Rent Assistance (RLRA) recipients who are transitioning out of or who have disengaged from Assertive Community Treatment (ACT) and intensive case management (ICM) levels of care. As these individuals navigate the loss of intensive, community-based wraparound supports, this role is critical to mitigating the elevated risk of housing loss. By applying trauma-informed de-escalation, rapid resource connection, and active landlord mediation, the Case Manager Senior resolves tenancy barriers, prevents evictions, and facilitates successful integration into independent community living. In all interactions, the position fosters a work culture of respect, trust, and understanding for the highly diverse populations served.
As the Case Manager Senior - RLRA Housing Retention duties will include:
Engagement, Housing Retention and Landlord/Neighbor Relations
Crisis Resolution and Stabilization: Perform complex, strengths-based case management to de-escalate severe housing crises and transition clients into long-term, stable housing.
Eviction Prevention and Mediation: Navigate severe lease violations and imminent eviction threats by negotiating stipulated agreements and cure-and-quit plans in partnership with property management and legal aid.
Clinical Engagement: Conduct consistent, face-to-face home and community visits to build therapeutic rapport, assess client well-being, and maintain ongoing connection to housing goals.
Strategic Liaison and Education: Serve as the primary advocate between tenants, landlords, and property managers, educating housing providers on the RLRA program and the Metro Supportive Housing Services (SHS) framework.
Proactive Tenancy Monitoring: Identify early warning signs of instability—such as behavioral lease violations or financial challenges—to intervene rapidly and prevent formal eviction proceedings.
Barrier Reduction and Capacity Building: Utilize flexible client assistance funds to resolve immediate physical or financial barriers while coaching tenants on essential independent living skills, unit hygiene, and lease compliance.
System Navigation, Benefit and Resource Connection
Consultation and Systems Navigation: Provide high-level guidance to partner agencies and external programs to effectively navigate the intersection of complex behavioral health systems and housing requirements.
Advanced Needs Assessment: Evaluate individual needs across multiple, overlapping systems to formulate and implement tailored, independent stabilization strategies.
Strategic Partnership Management: Cultivate and sustain collaborative relationships with community providers and agencies to ensure holistic, long-term support for clients.
Clinical Continuity Support: Proactively facilitate the re-establishment of behavioral health services to bridge gaps in care and sustain wellness for clients transitioning out of intensive programs.
Voucher Eligibility Maintenance: Ensure uninterrupted housing assistance by managing required RLRA voucher recertifications and maintaining strict program compliance.
Comprehensive Resource Connection: Advocate for and connect clients to essential financial and healthcare stability resources, including SSI/SSDI advocacy, SNAP benefits, supported employment, and Oregon Health Plan enrollment.
Referrals, Administrative, Collaboration and Data Collection
Complex Case Management: Manage highly complex cases across multiple systems and partner programs, utilizing advanced independent judgment to coordinate multifaceted risk mitigation and stabilize client housing.
Clinical Case Review: Conduct ongoing reviews of stabilized clients to evaluate progress, exercising clinical expertise to determine when to adjust monitoring levels or re-initiate intensive supports.
Data-Driven Evaluation: Analyze key programmatic data—such as eviction notices, risk mitigation funds deployed, and mediation outcomes—to evaluate intervention effectiveness and inform program improvements.
Systems and Pathway Development: Build and formalize streamlined referral pathways to ensure at-risk individuals are identified and enrolled, while assisting in the development of workflows, exit criteria, and program policies.
Documentation and Database Management: Maintain accurate, timely clinical and administrative records, ensuring seamless data entry and management across county, regional, and state databases (e.g., HMIS/ServicePoint).
Interdisciplinary Collaboration: Participate in bi-monthly cross-case conferencing and maintain active, collaborative partnerships with internal behavioral health teams and the Homeless Services Department to optimize client outcomes.

Required Minimum Qualifications/ Transferable Skills*:
Education/Training: Equivalent to an associate's degree course work in social science, social work, behavioral science, human services, or related field; AND, Three (3) to four (4) years experience that demonstrates the ability to perform the duties of the position.
Certification: Possession of and valid registration as a Qualified Mental Health Associate (QMHA) certification with the Mental Health & Addiction Certification Board of Oregon (MHACBO ) - more information can be found at MHACBO website .

Valid Driver’s license (Required).

Preferred Qualifications/Transferable Skills:
RLRA experience providing case management services and housing assistance
Prior experience working within, or closely alongside, intensive behavioral health teams (such as ACT, ICM, or mobile crisis response).
Working knowledge of Oregon Landlord-Tenant law, Fair Housing laws, and reasonable accommodation requests.

Hybrid/Highly Field-Based: At this time, this position is designated as hybrid; however, please note this is a heavy field-based role. While you may use your home as an administrative base up to two days a week, you should expect to be out in the community a large portion of your work week providing on-site, in-person services and home visits. A designated “remote” day is often used as a starting location for field work rather than a strict desk day. The designation of hybrid may be subject to change at a future time. All employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations. The onsite work location for this position is Five Oak building 209 SW 4th Ave, Portland, OR 97204.
