Job Description
Registered Nurse - Home Health

Schedule

Day shift starting at 08:00, working 5 days a week.

Assignment Details

Caseload of 5-6 patients per day, with 1 full weekend and 1 additional weekend shift required every 6 weeks. This role follows an hourly pay model, and travelers must start on time as stated in their contract.

Requirements

• Minimum of 2 years of experience in home health with high volume case loads.
• Active nursing license required at the time of submission.
• BLS certification required.
• Recent experience with OASIS is required; candidates must pass an OASIS exam prior to interview.

Responsibilities

• Provide nursing care to patients in a home health setting.
• Conduct assessments and develop care plans based on patient needs.
• Collaborate with interdisciplinary teams to ensure quality patient care.
• Document patient progress and maintain accurate records.
• Educate patients and families on health management and care plans.
• Implement and evaluate patient care interventions.

Compliance / Notes

No on-call requirements. Mileage reimbursement will be provided based on IRS standards for travel within a specified radius.
