RN Patient Care Navigator - Stroke Program

Hourly Pay Range: $40.45 - $62.70 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. Position Highlights Position: RN Patient Care Navigator – Stroke Program Location: Evanston Hospital Per Diem (As Needed) Hours: Per Diem (as needed) for 12‑hour shifts 7:00AM-7:00PM, must be flexible on days What you will do Render professional nursing care to assist physician and APP providers in managing the care of patients who present with signs and symptoms of acute stroke. Skilled in treating deteriorating and emergent patient conditions. Employ critical thinking skills, assess patient conditions, recognize deviations, implement appropriate treatment protocols for specific emergencies, evaluate patient response, record observations and interventions, assist with transport to emergency testing and transfer to higher level of care as indicated. Collaborate with stroke coordinator and neurosciences team to ensure care delivery meets comprehensive stroke center standards. Deliver education and serve as a resource for patients diagnosed with stroke and their care partners.
