• Setting: Acute care hospital case management
• Start Date: 7/28/2026
• Duration: 12-week travel assignment
• Schedule: Day shift with rotating weekends (every 3rd weekend)
• Shift Duration: 8-hour days
• Shifts Per Week: 5
• Hours Per Week: 40
• EMR: Epic
• Caseload: Typically 18-22 patients, with higher volume as needed

Job Requirements

• At least 1 year of RN case management experience required
• Strong bedside nursing and clinical practice knowledge required
• Experience managing complex patient populations in an acute care setting
• Active Registered Nurse license in good standing
• Experience with Epic EMR preferred
• Behavioral health experience is a plus
• Ability to work effectively in a collaborative, union environment
• Excellent communication, care coordination, and interdisciplinary teamwork skills

Responsibilities

• Coordinate discharge planning and case management services for patients across multiple units
• Manage a caseload of complex patients while supporting efficient transitions of care
• Collaborate with physicians, bedside nurses, social workers, and interdisciplinary teams to develop patient-centered care plans
• Assess patient needs, barriers to discharge, and post-acute care requirements
• Support timely progression of care and appropriate resource utilization
• Document accurately and efficiently in Epic
• Provide clinical insight and advocacy to help ensure safe, effective patient outcomes
• Adapt to changing census and staffing needs in a high-capacity hospital environment
