You'll provide skilled nursing care to both Home Health and Hospice patients in their homes while having the flexibility of an as-needed schedule and the support of an experienced interdisciplinary team.Schedule & TerritoryPRN – As NeededMinimum availability of two shifts per month. Flexible 4-, 8- and 12-hour shift opportunities are available, including day and PM coverage and weekday/weekend opportunities.Weekend/on-call commitment includes either every third weekend, working 8-hour shifts Saturday and Sunday, or participation in an on-call rotation approximately two to three times per month.You'll care for both Home Health and Hospice patients throughout Waukesha County and portions of surrounding counties. You'll coordinate visits directly with patients and caregivers and use your clinical judgment to manage your day while responding to changing patient needs.Your care may include nursing assessments, wound and ostomy care, medication education and management, injections and IV therapy, chronic and complex disease management, patient and caregiver education, and comfort and symptom management for Hospice patients. You'll communicate changes in condition, collaborate with physicians and other disciplines, and document care in Epic.What You'll NeedCurrent Wisconsin Registered Nurse license, BLS certification or ability to obtain, a valid driver's license and acceptable driving record, and the ability to independently prioritize patient needs while working in a mobile, field-based environment.Previous clinical nursing experience is preferred. Home Health, Hospice, case management or discharge-planning experience is helpful, but nurses with strong transferable experience in acute care, med/surg, wound care, IV therapy, infusion, oncology, rehabilitation and other hands-on clinical settings may also be a strong fit.Because care is provided in patients' homes, you'll need to be comfortable working in a variety of home environments that may include pets, tobacco smoke and common household allergens.New to Home Health & Hospice?You don't necessarily need previous Home Health or Hospice experience. Nurses coming from other clinical settings often already have the assessment, education, critical-thinking and hands-on skills needed for home-based care.
