Visit RNs then coordinate directly with their patients to organize their visits throughout the day.The department is based at ProHealth's Riverwood Administration Building in Waukesha, but this is a field-based role. Visit RNs travel throughout ProHealth Home Care & Home Hospice's service area based on patient needs.What You'll DoProvide skilled nursing care to Home Health and Hospice patients in their homesPerform assessments and monitor changes in patient conditionProvide wound and ostomy care, IV therapy and infusion careSupport symptom and pain management for Hospice patientsEducate and support patients and caregiversCollaborate with RN Case Managers, physicians and the interdisciplinary care teamDocument patient care in EpicVisit RN productivity is based on weighted units that account for the type and complexity of care provided, not simply the number of patients seen per day.What You'll NeedCurrent Wisconsin RN license or eligible Nurse Licensure Compact licenseValid driver's license in good standing and reliable transportationApproximately two years of professional RN experience preferredPrevious Home Health or Hospice experience is preferred, but experienced RNs with strong transferable clinical skills are encouraged to apply.
