This role focuses on assessing psychosocial needs, connecting patients and families to community resources, and supporting overall care plan goals to improve patient outcomes and quality of life.Key ResponsibilitiesConduct initial and ongoing psychosocial assessments of patients and familiesDevelop and implement individualized care plans in collaboration with the interdisciplinary teamProvide emotional support and counseling to patients and caregiversAssist with community resource coordination, including financial, housing, and support servicesSupport discharge planning and long-term care planningEducate patients and families on available resources and coping strategiesMaintain accurate and timely documentation in accordance with company and Medicare guidelinesParticipate in case conferences and care team meetingsQualificationsMaster’s Degree in Social Work (MSW) from an accredited programCurrent Florida licensure (LCSW preferred, MSW considered based on experience)Minimum of 1 year of healthcare experience (home health experience)Strong knowledge of community resources and discharge planningExcellent communication and interpersonal skillsAbility to work independently and manage time effectivelyValid driver’s license and reliable transportationAdditional InformationCompetitive per-visit ratesFlexible PRN scheduling based on your availabilityOpportunity to make a meaningful impact in your local community #J-18808-Ljbffr Senior Care Solutions Inc
