About the Role:
We are seeking a compassionate and skilled LPN to join our team. As an LPN at Green Palms Health and Rehab of St. Marys, you will have the opportunity to work collaboratively with our interdisciplinary team to deliver individualized care to our residents. Your primary responsibilities will include:

• Administering medications and treatments as prescribed by physicians

• Monitoring and reporting resident changes and concerns to the healthcare team

• Assisting with daily living activities, such as bathing, dressing, and grooming

• Providing emotional support and comfort to our residents and their families

• Maintaining accurate and up-to-date resident records

Responsibilities:

• Provide direct patient care under the supervision of a registered nurse

• Develop and implement individualized care plans for each resident

• Collaborate with other healthcare professionals to ensure comprehensive care

• Maintain a clean and safe environment for our residents

• Participate in quality improvement initiatives to enhance resident outcomes

Requirements:
To be considered for this role, you must have:

• A current LPN license in the state of Ohio

• High school diploma or equivalent required

• 0 years of experience in a long-term care setting

• Ability to work a varied schedule, including days, evenings, and weekends
