Asistente de Servicios Clínicos

Responsabilidades Asiste a los clínicos en la prestación de servicios coordinados y gestión de casos para clientes adultos con problemas de salud mental y/o abuso de sustancias. Todos los empleados en esta posición son considerados 'Empleados Esenciales' y pueden ser requeridos para proporcionar cobertura de turno mandada para necesidades de programación. Utiliza recursos comunitarios y hace referencias apropiadas en apoyo del Plan de Tratamiento del cliente y los planes de atención continua después del alta. Ayuda con los arreglos de transporte, según lo aprobado por el Gerente del Programa, para que los clientes asistan al programa y cumplan con los aspectos de su Plan de Tratamiento que no pueden ser atendidos en el lugar. Se conecta con recursos del área a través de agencias de servicio, salud y empleo, grupos profesionales, comités y consorcios para adquirir un conocimiento integral de los servicios disponibles para los clientes. Participa en conferencias de tratamiento, reuniones de personal y capacitación interna para apoyar la prestación de atención de alta calidad al cliente.
