Physical Therapist Assistant - Full Time

We are seeking a Physical Therapy Assistant to provide skilled education, treatment, and interventions in a home health setting, following the Physician's plan of care. Key Responsibilities Include Delivering direct physical therapy services in accordance with directions from the physical therapist and physician. Communicating regularly with patients and caregivers about the care plan. Providing treatments, procedures, and education to ensure patient safety and optimal function. Reporting patient observations and changes in condition to supervisors and physicians. Teaching patients, families, and caregivers on the safe use of prosthetic, orthotic, and assistive devices. Collaborating with interdisciplinary team members to discuss and plan patient care. Planning for discharge and providing relevant instructions to patients and caregivers. Maintaining documentation, clinical notes, and ensuring confidentiality per professional standards. Participating in in-service programs and other job-related duties as assigned. Qualifications Include Graduation from an accredited Physical Therapist Assistant program. Current, unrestricted state licensure. Valid CPR certification. Professional and ethical conduct, with adherence to confidentiality and documentation standards.
