Social Worker Part time, 24 hours/week Requirements: Bachelors in Social Work or a related field Job Summary We are seeking a highly motivated and compassionate Social Worker to join our team. As a Social Worker at AdviniaCare Summit Commons, you will play a vital role in assessing residents' needs, developing care plans, and facilitating discharge planning. Responsibilities Assist in assessments, including Admission Assessments, Trauma assessments, and other evaluations as needed Attend Care plan meetings and contribute to the development of individualized care plans Assist in discharge planning, ensuring a smooth transition for our residents Collaborate with the Director of Social Services to address resident needs and concerns Support residents and families with emotional and psychological challenges Maintain accurate and detailed records of assessments, care plans, and resident interactions About the Role As a Social Worker at AdviniaCare Summit Commons, you will have the opportunity to make a meaningful difference in the lives of our residents.
