Hybrid EI Care Navigator: Family Intake & Referrals
State of Colorado
The Early Intervention (EI) program in Colorado coordinates developmental services for children from birth to age two with special needs, using referrals via multiple channels and a statewide Salesforce system. This role is the first point of contact for families, guiding them through evaluation access and program rights, and scheduling multidisciplinary assessments.
