Experience with HMOs, IPAs, assessing patients, care coordination, and discharge planning. Ability to work with computerized documentation systems. Commitment to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day. Description: This position is for a registered nurse specializing in case management. The role involves managing patient care coordination and discharge planning. The nurse will be responsible for a caseload of up to 25 patients. Weekend coverage is required, typically one full weekend every four weeks, subject to departmental needs. The position is open to first-time travelers but requires screening. The candidate must have experience with care coordination in an acute medical setting. This is a day shift position with hours from 08:00 to 16:30. The environment includes working with adults and geriatrics. The hospital setting emphasizes adherence to health guidelines and regulations such as HIPAA. The success in this role depends on strong organizational and communication skills. The case manager will also be expected to collaborate with interdisciplinary support teams to facilitate patient care. Floating between facility locations may be required.
