PROVIDER MUST HAVE A CLEAN FILE

• Clean files must include a clean NPDB, clean malpractice (dismissed and dropped cases as well), and a clean background check.

NOTE a July or early August start will be contingent on approval from the medical staff office for temporary privileges. We can look more towards a mid August start ongoing if not approved.

• Hours Worked for OT to apply: 8 per day
• Rates to be Filled Out on Bid: Clinic and Call: Regular, Orientation, Overtime, Holiday, Night Call, 24 Hour Call, Call-Back.

ADDITIONAL DETAILS

Only looking for 1 provider for coverage. We have open shifts in July and August, as well as will need ongoing gap coverage.

• July 13th: Day , July 14th Day and Call, July 15th Day, July 16th Day and Call, July 17th Day. (Please refer to note above about approval for privileges being contingent)
• August 3rd Day and Call, August 4th Day, August 5th Day and Call, August 6th Day, August 7th Day and Call, August 8th Day and Call, August 9th Day and Call, August 29th Day and Call, August 30th Day and Call.

Ongoing shifts will be available.

Floor shifts are 7:30 A to 4P with 30-minute unpaid lunch
On call: 6pm to 7A next day (beeper call)
Start time may vary, depending on scheduled cases. Shifts are too be billed as regular hours and night call on both weekdays and weekends.

Responsibilities will be: Rounding on inpatients post-op, first assist in surgery, seeing patients in clinic

Skills required: Endoscopic and radial artery harvesting, interpret EKGs, X-rays, Labs, ECMO; IABP and Impella
