Skilled Nurse PositionRapidly growing! Urgently hiring!The Skilled Nurse performs in accordance with physician orders and the plan of care under the direction and supervision of the RN. Services are furnished in accordance with Agency policies and procedures. We offer the following to our employees:Be part of a unique healthcare company where we can help those in our communities who need our help the mostFlexible hours/work-life balance (full-time or part-time available)Competitive payPaid on a weekly basisMedical/dental/vision/life insurancePaid holidays/401(k) matchCareer growth opportunitiesGreat and collaborative work environmentResponsibilitiesWhat You Will Do:Administer medication in accordance with standards of practice.Encourage social/rehabilitative skills and encourage self-care and independence.Perform patient treatments in accordance with the plan of care.Assist the patient in learning appropriate self-care techniques.Prepare clinical and progress notes to document care providedMonitor, document accordingly and report any change in patient condition to the Clinical Director.Promote activities to achieve a safe and healthy environment.Observe aseptic technique when performing patient procedures/care.Practice Standard Precautions at all times.Work with other members of the clinical team to develop and improve the plan of care.Assist the patient in achieving established goals in the plan of care developed by the RN CM.Maintain productivity according to the standards of the agency.Participate in patient care meetings, in-services, orientation and staff meeting.Understands when to request supervisory assistance.Adhere to the policies and procedures of the agency.Provide service in accordance with agency's philosophy, standards, policies and scope of practiceParticipates in Performance Improvement and adheres to HIPAA guidelines.Other duties as assigned.QualificationsLicensed nurse in the State of Ohio, in good standing.Current BLS CPR CertificationOne year or more experience as an RN/LPN in home health preferred.Must be able to read and understand various documents used by the agency including plan of care, flow charts, patient charts, agency policies and procedures.Knowledge of Medical Terminology is required.Must be able to use a computer, laptop or tablet.Requires math skills including ability to add, subtract, multiply and divide whole numbers.Employees must always practice patient confidentiality to protect the patient's privacy.Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.Employees must promote customer service and always provide exceptional patient care, doing whatever we can to please our patients while staying within the rules of the agency and rules of each program served.Type and computer skills are required for this position.Eligibility for certain benefits may depend on employment statusHomecare with Heart is an equal opportunity employer committed to providing equal employment opportunities without regard to race, color, religion, sex (including pregnancy), sexual orientation, age, national origin, disability, genetic information, veteran status, or any other classification protected by applicable law.
