*Nurse Case Manager II - New Jersey

A-Line Staffing is hiring Nurse Case Manager II in Brick Township, NJ 08723 (field territory in Northern NJ counties; work-from-home when not traveling). OVERVIEW Provide RN case management for dual-eligible members, completing assessments and care plans, coordinating services, and traveling locally for in-person member visits. Compensation $44.14 per hour Benefits 401(k) with company match after 1 year of service (on eligibility dates) Highlights 26-week contract assignment (could extend or go direct based on attendance, performance and business needs) Schedule: Monday–Friday, 8:00 AM–5:00 PM Work from home flexible: Fully remote (never coming onsite) when not traveling Travel requirement: 25–50% travel within a 50-mile radius using your personal vehicle (mileage reimbursed) Territory: Must be able to cover counties including Essex, Passaic, Hudson, Sussex, Morris, Bergen (NJ) Responsibilities Assess, plan, implement, and coordinate case management activities to support member wellness. Use clinical tools and data review to evaluate member needs, benefits, and care gaps. Apply clinical judgment to reduce risk factors and address complex medical and social needs impacting care planning. Conduct comprehensive assessments using multiple sources (including claims) to address co-morbidities and functional impacts. Make referrals to clinical resources and interdisciplinary team members as needed. Collaborate with supervisors and key stakeholders; present cases in interdisciplinary case conferences. Follow regulatory and company case management policies and procedures. Use motivational interviewing to maximize member engagement and identify health needs. Requirements Active, unrestricted RN license in New Jersey Minimum 3+ years of clinical practice experience Verifiable High School Diploma or GED Must reside close to or within: Essex, Hudson, Bergen, Passaic, Sussex, or Morris Counties (NJ) Able to travel 25–50% locally using reliable personal transportation Comfortable working independently in a work-from-home setting and collaborating virtually Basic computer skills and ability to navigate multiple systems (MS Outlook/Word/Excel, etc.) Attendance is mandatory for the first 90 days Preferred Qualifications Certified Case Manager (CCM) preferred 2+ years in care management, discharge planning, and/or home health care coordination preferred NJ Choice Certification preferred Bilingual preferred Strong analytical, problem-solving, organizational, and communication skills Job ID: 175361 #J-18808-Ljbffr your Jared
