The LPN is responsible for providing direct nursing care, administering medications and treatments, monitoring resident conditions, and collaborating with the interdisciplinary team to ensure exceptional resident outcomes.## Key Responsibilities* Provide direct nursing care in accordance with physician orders and individualized care plans* Administer medications and treatments safely and accurately* Monitor residents' conditions and promptly report changes* Maintain accurate and timely clinical documentation* Supervise and support Certified Nursing Assistants (CNAs)* Communicate effectively with residents, families, physicians, and interdisciplinary team members* Respond appropriately to resident needs, emergencies, and changes in condition* Promote resident dignity, comfort, and quality of life* Ensure compliance with all state, federal, and facility regulations* Utilize PointClickCare (PCC) and other clinical systems as needed## ## Qualifications* Current, unrestricted **LPN license in South Carolina*** Current CPR certification* Must maintain all required continuing education and licensure requirements* Experience in skilled nursing or long-term care preferred* Experience with **PointClickCare (PCC)** preferred* Strong clinical assessment, communication, and organizational skills## What Makes You a Great Fit* Compassionate and resident-focused* Dependable and professional* Strong teamwork and leadership skills* Excellent communication and documentation abilities* Passionate about providing high-quality care to an underserved population**Apply today and join a team where your contributions are valued, your growth is supported, and your care makes a difference every day.**We are an equal opportunity employer.
