Behavioral Health Specialist
Capital Health Plan
Overview Supports members by conducting timely assessments and care plan development following a patient's emergency room or acute care discharge and educates members on available resources. Clinical interactions with the members bridge clinical expertise with care management to improve member outcomes and reduce costs through navigation through the healthcare system. Minimum Qualifications Master's degree in social work or related field and at least three years' related experience. Fully licensed as a Florida Licensed Clinical Social Worker (LCSW) or Florida Licensed Mental Health Counselor (LMHC). A minimum of four years of related experience is preferred. Experience in managed care is preferred.
