Responsibilities for Licensed Social Worker (LSW) Evaluate patients (existing, new, and prospective) based on their psychosocial needs, strengths, and limitations. Address patient concerns and goals while maintaining consistent communication with patients, families, and case managers. Build and maintain strong, supportive relationships with patients and their families to promote trust and engagement in care. Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments, to support comprehensive care planning. Develop, implement, and update individualized care plans that reflect resident needs, goals, and progress. Advocate for residents’ rights, emotional well-being, and access to appropriate services and resources. Support discharge planning and transitions of care in coordination with the facility and external partners. Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements. Ensure compliance with all applicable laws, regulations, and standards governing social services in post-acute care settings. Qualifications for Licensed Social Worker (LSW) College degree in Social Work or related field preferred. Must have prior long-term care experience. Ability to make independent decisions when circumstances require sound clinical and ethical judgment. Strong knowledge of social work practices, procedures, and applicable laws, regulations, and guidelines governing post-acute care facilities. Ability to implement and interpret departmental programs, goals, objectives, policies, and procedures. Proficient in social services responsibilities, patient rights, safety, and sanitation standards. Demonstrates integrity and professionalism by adhering to the Marquis Code of Conduct and completing all required compliance training.
