REPOST APS Case Manager

Special Notice: This is a REPOST – Previous applicants need not re-apply for this position. Job Duties The APS Case Manager performs specialized case management in the area of Adult Protective Services (APS), Division of Adult Advocacy, in the following county: Williamsburg. Case management includes but is not limited to conducting investigations, conducting assessments and coordinating protective and preventive services for vulnerable adults. Investigates to determine vulnerability, abuse, neglect by caregiver, self-neglect and exploitation reports of vulnerable adults while respecting self-determination of vulnerable adults. Will assist other counties with APS case management as needed. Provides case management services according to APS policy and procedures to include home visits and legal proceedings and enter documentation into CAPSS as required in a timely and accurate manner. Coordinates necessary services to keep the vulnerable adult safe to include temporary and long-term placements. Assists law enforcement and/or family court in emergency removal of adults, and secure placement in a protective environment. Maintains professional and harmonious relationships with co‑workers and the community. Assesses the vulnerable adult's ability to secure long‑term services to include but not limited to redirecting funds, securing a guardian or conservator, filling out appropriate documents for emergency services and connecting to probate or family court. Ensures that Personal Identifying Information (PII) is properly used, accessed, gathered, shared and disposed. Protects the agency networks and applications by safeguarding systems, equipment and data. Performs Red Cross shelter duty or other emergency shelter duty/services as directed. Performs other duties as required and/or directed. Minimum and Additional Requirements A Bachelor's Degree in Social Work, Human Services, Public Administration or a related field. A Bachelor's Degree in any other field and one (1) year experience in a related field. Must possess a valid driver’s license and have access to and be able to operate a motor vehicle; individuals must comply with necessary field‑work assignments. Position will be located in Williamsburg. If selected for a job offer, a certified official transcript must be provided within two (2) weeks of employment. Division Adult Protective Services / Williamsburg County. State employees may also be eligible for other benefits, including tuition assistance; holiday, annual and sick leave; and discounts on purchases, travel and more. Note: The benefits above are available to most state employees, with the exception of those in temporary positions.
