RN Transplant Coordinator - Bone Marrow Transplant Program

The Bone Marrow Transplant and Cellular Therapy (BMTCT) Transplant Nurse Coordinator coordinates allogeneic and autologous stem cell transplants, as well as cellular and gene therapies (e.g., CAR-T) for pediatric and adult patients. The coordinator provides comprehensive care across the continuum—from pre-transplant evaluation through transplant and long-term follow-up. This role involves collaboration with a multidisciplinary team to ensure optimal patient outcomes, education of patients and caregivers, and adherence to regulatory standards. The coordinator functions as a clinician, educator, investigator, consultant, and leader, applying evidence-based practice and clinical judgment to solve complex problems and support excellence in patient care, research, and education. Key Responsibilities Program Development & Planning Collaborates with the BMT program administrator in strategic planning and implementation of program goals through regular team meetings and quality initiatives. Continuous Quality Improvement (CQI) Participates in CQI efforts to optimize patient and graft outcomes. Reviews clinical practices and patient resource utilization to identify areas for improvement. Contributes to the development and refinement of clinical pathways and protocols. Education Educates patients and families on the transplant and cellular therapy process, including expectations, risks, and post-treatment care. Develops and delivers educational materials for patients, staff, and external stakeholders. Patient Care Coordination Manages the clinical, psychosocial, and financial aspects of care for transplant and cellular therapy patients. Coordinates pre-transplant evaluations, donor searches, and treatment planning. Oversees outpatient care. Regulatory Compliance Maintains accurate and timely documentation in the electronic medical record to ensure compliance with FACT, and other regulatory bodies. Supports data collection and reporting for internal and external audits. Preference given to applicants with pediatric hematology/oncology and/or pediatric BMTCT experience. Summary This position coordinates solid organ and bone marrow transplantation of pediatric and adult transplant recipients. This position provides care across the continuum from pre-transplant, during transplant and post transplant. This position plays a critical role in managing the complex processes of transplantation and other therapies. They act as a liaison between patients, medical teams and procurement organizations, ensuring smooth communication and coordination of care. Responsibilities Patient Care Coordination Coordinate with multidisciplinary teams to develop and implement an individualized plan of care for pre- and post-transplant patients. Oversee and/or coordinate the clinical, psychological and financial needs of the patient. Coordinate pre-procedure tests and consultations to determine transplant or therapy candidacy. For solid organ transplants, manage the program’s waiting list, including regular follow-up on patient status and coordinating organ offers. Facilitate the transplant process by organizing the patient and the medical team. Monitor inpatient status, discharge needs and post-transplant follow-up care. Coordinate care across outpatient and inpatient settings. Assess organ function and general health status, determining the need for additional assessment or intervention. Education Educate patients and their families about the transplant/ therapy process and what to expect during each phase. Plan, acquire, or design educational materials and present educational programs to patients, staff, other professionals, the community and outside agencies. Educate nurses and other multidisciplinary team members about transplant care to promote positive outcomes. Regulatory Compliance/Quality Improvement Document and maintain patient information in the electronic medical record with current data and required information to ensure regulatory compliance with Centers for Medicare & Medicaid Services (CMS), Organ Procurement and Transplantation Network (OPTN), Foundation for the Accreditation of Cellular Therapy (FACT) and/or Center for International Blood and Marrow Transplant Research (CIBMTR) guidelines as applicable. Collaborate with transplant leadership in program planning. Participate in the development, implementation, and evaluation of the program’s Quality Assurance and Performance Improvement (QAPI) efforts. Review patient resource utilization and identify areas for improvement. Participate in the development and improvement of clinical workflows Other Information Education Requirements: Bachelor's degree in Nursing (BSN) from a state-accredited school of professional nursing. Licensure/Certification Requirements: Licensed to practice as a Registered Nurse in the state of North Carolina. Related certification may be required per individual department guidelines: For Solid Organ Transplant: Certified Clinical Transplant Coordinator (CCTC) is strongly encouraged within three years of hire. For Bone Marrow Transplant: Either Oncology Certified Nurse (OCN) or Transplantation and Cellular Therapy Certified Nurse (TCTCN) certification required within two years of hire. Professional Experience Requirements: Minimum of 3 years of nursing experience in a clinical setting. Solid Organ Transplant: transplant and/or critical care experience preferred. Bone Marrow Transplant and Cellular Therapy Program: hematology/oncology experience preferred. Pediatric hematology/oncology and/or pediatric BMTCT experience preferred for this role. Knowledge/Skills/and Abilities Requirements: Excellent communication and interpersonal skills to effectively engage with patients and healthcare teams. Strong organizational ability, attention to detail and proficiency with electronic medical records.
